KMM is coordinating a health fair for MUNTER'S CORPORATION scheduled for THURSDAY, OCTOBER 16, 2025  1PM - 3PM.  If you received this invite, it is because they requested your services, information, and participation in their upcoming event.  This event is going to book fast since there are limited tables, so if you are 100% certain you are able to attend, register.

• Table size for your booth is approx. 4-6 foot.  You will be allotted 2 chairs as well.  If you need specific set up requirements, please indicate in registration form below.
• Set up time for providers and vendors is between 12PM - 12:30PM
• Health insurance information: BCBSMA PPO   Dental: METLIFE  Vision: EYE MED
• You will receive a confirmation email with an attached confirmation letter.  If you do not receive a confirmation email within 24-hours, please email lenore@kmmtx.com.
• THERE IS NO COST TO PARTICIPATE.  A DOOR PRIZE IS REQUIRED UNLESS YOU ARE A NON-PROFIT ORGANIZATION.
• Demographic breakdown % of male to female:  25% men  75% women

• Company has 225 employees; expected attendance is 150; 70% have children
• PARKING:  Available parking in the Munter's visitors parking and any available parking spaces available on the premise.
• The health fair will be held  in the company breakroom.
