Information on company/health fair:

• Health fair will be held in the company cafeteria
• Table size is 4 x 4 and 2 chairs will be allotted per table
• Company has 506 employees.  They expect over 400 to attend the health fair.
• 65% of the employees are male / 35% of the employees are female
• 30% of the employees have children under 18.
• Company Insurance: Medical - UMR PPO  Dental - UMR  Vision - VSP
• Parking - There is parking available for vendors in front of building; park where available
• Door prize required unless certified nonprofit organization.
